Limited Time Offer. To qualify for the Risk Free 30-Day Trial, you must (1) order a bundle of Grande phone, cable and high-speed internet services before December 31, 2008, (2) be a first-time customer of a Grande three service bundle of phone, cable and high-speed internet services, (3) pay the monthly recurring service fee for the Grande three service bundle for one month of services plus all other amounts payable to Grande, (4) cancel all three Grande services within 30 days of installation, and (5) within 30 days of service cancellation, return all cable modems and digital receivers to Grande and deliver to Grande a written request for cancellation under the Risk Free 30-Day Trial offer. The total maximum amount that will be paid or credited under the Risk Free 30-Day Trial to a single qualified household will be an amount equal to one month’s monthly recurring bundled service fee for Grande phone, cable and internet services plus any installation fee and any separate cable modem charge paid by the customer. Any such payment or credit will made within 60 days after all conditions for the Risk Free 30-Day Trial are satisfied. The Risk Free 30-Day Trial does not apply to and no refund or credit will be given for any usage-based charges such as charges for pay-per-view movies or events, long distance calls, directory assistance, directory assistance call completions, calling card calls, operator-handled calls and inbound toll-free calls.
